Quality and Leadership
in Supports for
People with Disabilities
(the Council)'s
"Personal Outcome
Measures" help
to keep the focus
on each person
as the key decision
maker in his or
her own life.
This manual begins
with an overview
of the connection
between planning
and the Council's
"Personal Outcome
Measures".
Personal Outcomes
are what people
expect from the
services and supports
they receive.
Personal Outcomes
refer to the major
expectations that
people have in
their lives.

for using personal
outcomes in planning
individual supports
and services are
outlined in chapters:

Council has
established
25 "Personal
Outcome Measures"
which focus
on the person's
identity, autonomy,
affiliation,
attainment,
safeguards,
rights, and
health & wellness.
The manual walks
the reader through
the proposed
planning process
including: preparing
for the session;
ground rules,
facilitating
the session;
identifying
outcomes that
are most important
to the person;
communication:
without words,
tips for effective
listening, developing
and answering
questions; etc.
